Policy: Injuries On-Duty

Purpose:
The purpose of this policy is to establish guidelines to direct the actions of University of Wisconsin-Platteville Police Officers who are injured while “on-duty” for the University of Wisconsin-Platteville Police Department.
Policy:
Any and all injuries or illnesses that occur to any staff in the University of Wisconsin-Platteville Police Department, while on duty, should be reported to a supervisor immediately. The UW-Platteville Police Department will follow the rules and guidelines provided by the Universities of Wisconsin.

Procedures:
- In the event that an employee sustains injury or illness while on-duty, the employee will initiate the work-related injury procedures found on the Universities of Wisconsin Website.
- Notify a supervisor as soon as possible, and start the paperwork found on the forms shelf in the supply room, in a binder labeled, “Workers Compensation”.
- The employee is responsible for completing the following forms contained in that binder:
- Employee’s Work Injury and Illness Report
- University of Wisconsin System Office of Safety and Loss Prevention Worker’s Compensation Program – Authorization to Use or Disclose Health Information to Worker’s Compensation Self-Insurer
- A supervisor will complete the following forms, also contained in the binder:
- Employer’s First Report of Injury or Disease
- Supervisor’s Accident Analysis and Prevention Report
- Supervisor’s Evaluation of Repetitive Motion and/or Materials Handling Activities
- Return to Work Evaluation
- The employee is responsible for getting the following documents completed by the physician, also found in the binder:
- Return to Work Evaluation
- All forms found in the binder described above must be returned to a supervisor who must physically (hand) deliver the binder to the Payroll and Benefits Specialist within 24 hours of the injury.
