Student Laptop Checkout - Baraboo

Overview

Description

Students of UW-Platteville at Baraboo are able to checkout a laptop from the ITS Help Desk for one week at time for various reasons. Students may need access to campus software that they may not have on their personal computer, their personal computer is not functional, or student groups going to conferences/competitions. This form is to be filled out at the Help Desk (A107 on the Baraboo campus), if no one is present check in room A106 or A108, before the laptop is checked out to the student. If we receive requests for laptop checkouts and the student is not present when filling out the form in our office, we will cancel the request.

Information needed for Request to be Completed

  • Requestor's Name
  • Any specials requests (software needs or extended checkout requirements) - ADD to Description field
  • Checkout Date - Defaults to today's date
  • Return Date - Defaults for 7 days from today (ITS staff can adjust if needed)

Eligibility

UW-Platteville students are allowed to fill out this form. If an advisor or department needs to checkout a laptop(s) for a student event please contact the ITS Help Desk and a staff member will further assist with your checkout request.

Fines maybe assessed if the laptop is returned late, unexpected damaged, missing accessories, or cleanliness. A full list of the fines can be found here.
