Description
This form will be used to request access to an email address or listserve. The owner MUST submit this form. If the owner does not submit the form there will be a delay in the processing of the email access request. If the owner needs to be changed for a specific email account or listserve please let us know in the description field of the form.
Information needed for Request to be Completed
- First Name
- Last Name
- netID/username
- Name of Email Address or Listserve
How to get this Service
To make this process easier, use the associated Excel file listed under Attachments. Download and fill out the Excel file with the required information. Once the Excel file has all the needed information click the Request Service button and attach the Excel file to the form for submission.
