Information needed for Request to be Completed

  • Requestor Name
  • Requestor Department
  • Instructional Course SIS ID or other course identifying information
    • SIS ID
    • Course Catalog Number
    • Department, Discipline
  • Course Announcement Text
    • Within the service request form there is example text of what the the announcement could say
  • Survey Link
    • Link provided to capture survey results
  • Delay Announcement Until (Date)
    • Date that you would like the announcement to start displaying for students to fill out the survey
    • If no date is entered, the announcement will be displayed immediately after a member of the Digital Learning team has processed the request
