Overview
Description
This new Remove Access Request form combines many of the access types that are granted through the ITS Department. This new form has been created to hopefully make the process of removing access for employees easier. Also only needing to fill out one form verses two or more forms when access needs to be removed. This form will be used to remove access to the following: Calendars managed within Outlook, Email Accounts, Okta Applications, Printers, Rooms managed within Outlook, SharePoint, Teams, and WorkGroups.
Information needed for Request to be Completed
Each type of access request requires different information, below you see what is needed broken down by type.
ALL access request types require the following information:
- Requestor
- Account/Department
- Individual(s) Needing Access - This is now a searchable field
- What Type of Access - This is a multi-dropdown that will let you select multiple access types
- Additional Information - Not Required
- Attachments - Not Required
Calendar
- Calendar Address/Name (ex: CAL_ITS Staff Calendar or cal_itsstaffcalendar@uwplatt.edu)
Email
- What type of Email Access
- Email List (to receive messages from email list)
- Email List Address (ex: faculty-l@uwplatt.edu)
- Mailbox Access (delegate access, assumes FULL access to a shared mailbox)
- Email Address
- Email Address Access
- Send to Email List
- Send to List Address (ex: all-students-l@uwplatt.edu

Teams
Before submitting this request for Teams access removal, please work within your department(s) to update the access within Teams first. This should only be used when the owner of a team no longer has an active account with the campus and there are no other owners that could facilitate updating access.
WorkGroup
How to get this Service
Click the Request Service button. A ticket and separate task(s)s will be created for each type of access being requested. The requester will get notifications when the ticket/tasks have been created, when the individual tasks have been completed by ITS, and when ALL tasks have been completed which will also mark the ticket as resolved.

This form is available to Students, Faculty, and Staff
Service Owner
Determined by the type of access being removed.

There is no cost associated with this service.
