Add Workgroup Access

Overview

Description

This form will be used to request an addition to a pre-existing workgroup. The owner MUST submit the this form. If the owner does not submit the form there will be a delay in processing the workgroup request. If the owner needs to be changed for a specific workgroup please let us know in the description field of the form.

Information needed for Request to be Completed

  • First Name
  • Last Name
  • netID/username
  • Name or Path of WorkGroup
  • Type of Access (readonly or readwrite)

How to get this Service

To make this process easier, use the associated Excel file listed under Attachments. Download and fill out the Excel file with the required information. Once the Excel file has all the needed information click the Request Service button and attach the Excel file to the form for submission.

The owner of the WorkGroup must submit this request.
